Jerking after replacing clutch

Hi to all!

I own a Mazda 3 (1.6i engine) 2010 model.

I recently changed my engine's clutch after having some problems when shifting into gears (recommendation of my mechanic). Slave cylinder/fluid were also replaced.

Clutch fork, throw-out bearing, disc and pressure plate are new (from LUK).

After 2200 km with the new clutch, my car has the following symptoms:

- car jerks when accelerating in 1st, 2nd and 3rd (even more when a/c is on);

- poor shifting in 1st ,2nd 3rd gear;

- clutch's fork has a strange squeak/movement, as can be heard/seen in the following video: https://www.youtube.com/watch?v=F_2ImsHCIoU

- squeak noise when pressing/depressing the clutch pedal which comes out from the engine compartment; Clutch pedal does not feel right.

Also, when at stoplight i can feel a slight vibration in the steering wheel and hood. It feels the engine is moving too much and the engine mounts can't handle it. No leaking or broken engine mounts.

Any ideas ? Maybe the clutch kit was not installed properly ? Or my motor mounts could be worn?
