Do I need to replace my #4 engine (trans) mount?

2009 Mazda 5 Sport
I replaced the #3 mount a few months ago. I still get vibrations around the 60mph area and it seems to pull to the left. The updated TSB says to look at the #4 mount for vibration issues. I decided for kicks to take out the battery and battery box and take a look at it. Seems OK to me? Any way to determine if it is bad or not?

I should note this was in Drive, with the parking brake on, and reving the engine

Here are pictures of my transmission mount when it went bad. I had a lot of vibration in drive stopped at stoplights.

I looks like yours is pillowing out around the sides as it has collapsed. The sides should be straight like the new one I installed.

Do you remember if you used an aftermarket one or one of the OEM ones?
 
The failed one you see is aftermarket from Rockauto. It had just under 60k on it in this picture and was much much worse than the OEM one I pulled out at 80k. I replaced it with a new one from my local dealership as they had the same price as an online OEM Mazda parts retailer I found.
 
So I finally did this 11 days ago. I'm at 67k miles

It did help with the idle vibration. $70 after shipping from Tasca Auto
