ya, i can clearly hear it in the cabin while driving... however, since i can only usually hear it after it warms up, i get the idea that it is more of a top of engine thing... wouldn't something like a spun bearing make noise always, not just on start up? and its more of a tick than knock..

I've got an '02 with 141,000 miles. Recently over the past 2 months I've noticed an odd noise seeming to come from the engine... It occurrs: -after the engine is warmed up. -in every gear -noise increases in frequency with RPM -under any heavy acceleration in RPM over 3000ish...

What about the bushings within the shift linkage or the ones holding the shift lever in place? Could that affect just one gear? I just would think if the motor mounts were causing shifting problems it would affect more than one gear. How would this explain just 3rd popping out?

Is there a device that i can test fuel pressure with? Can a nice OBDII scanner give me detailed information on spark voltage and/or better info than just the cheap ones they let you borrow at autozone?

Yes, it is still misfiring under heavy load - not idle though. If i blip the throttle, i can feel hesitation that wasn't there before my problems started. And as for the MAF sensor, i keep the filter clean, nothing looks dirty.
