HID Ballast replacement

It's been so long since I've posted that I was looking for an "Electrical" section within Proteges until I realized it's considered an "old" Mazda now... ouch. :)

So I believe I need to replace one of the ballasts on my hid kit as one of the headlights turns off while driving at night. I have to turn the headlights completely off and then back on to get the headlight to come back on. It will do this multiple times while I'm driving. I don't think it's the bulb as it does come back on but I have switched the bulbs around to make sure.

It's a Philips Bi-Xenon H4 35W 6000k kit. I can no longer see any serial numbers or model numbers on the ballast to indicate what I should look for. That being said I have a few questions:

1. Provided I match the connections that are currently on the ballast, can I use that as an indicator of what type of ballast to buy?
2. Besides Philips, is there another brand that's recommended.
3. Is it best practice to replace both ballasts at the same time?
