did you check to see if the AC drain is clogged ? the fan makes noise when the blades hits the water, see this link for instructions:

if it's not a clogged drain and you have a faulty blower motor, you can buy an plug and play aftermarket replacement

from Amazon for $48 and fix it yourself, it's really easy, the blower is easily accessible behind and below the glovebox

Rock Auto has a standard fan assembly (motors, shroud, & fan blades) for $221. NAPA has them for $379. The heavy duty version for the trailer towing package costs more.

The noise and shaky fan blades sure sound like bad motor bearings. Change the motors, and the fan blades if the hubs are worn, or the whole drop-in assembly before it dies on you and the engine is possibly damaged by over heating. What about the cheap one?...I don't know, but if I was planning on keeping the car only a couple more years, I'd go cheap. If I planned on keeping the car longer, I'd get NAPA or something else possible better quality. Mazda parts from an on line vendor are $568 or $644.
