Went at my dealer. Cap was replaced for free. The new one is a bit different and is a better fit.

Got it for my Mazda 5 GT 2008: Hold all 3 for about 5 seconds: Radio power ON, preset 5 and seek down.

Found it: you need to hold down radio power button, preset 5 and seek down (all three at the same time for a little while until the display shows Passwd Cleared)

Resurecting an old discussion... Same problem here. The cap was stuck... unable to unscrew it off. Then, the top of the cap stayed in my hand while the other part was still in. Used crimps to take it off... But I realized it was a bit off; I surely screwed it in not perfectly perpendicular...

Same problem here. Got a 2008 Mazda 5 GT and bluetooth is locked with a PIN. I tried the "turn the ignition to ACC or ON, turn the radio off, then hold the Auto-M and Preset 2" but it doesn't work. Someone had a solution since Feb 2011? Thanks.
