How is it attached to the floor? Was all the necessary hardware included?

There are 3mm hex screws x4 + hex driver in the package. I used a hammer and a small nail to make holes. Just put it in last night, fit & feel is good.

I chose this particular pedal because its shape fits the Mazda5's dead pedal space. You can get some yellow, blue, or red versions for cheaper on eBay.

Match? Sort of - if I had the time I would paint it black!

It sometimes reflects light big time, and can be distracting when driving.

I think we're both suggesting to match by getting new brake and gas pedals with a similar look.
