What fittings does it come with (tube size, how many, caps)? Are the added ports female thread, if so, what thread? Do you have the fitting for the other end so it will fit on BB or straight-through vac line? Thanks.

The only thing that is included is the manifold and the end fittings... but the port fittings are 1/8 Pipe x 3/16 Barb.... with this you don't need caps you only drill what you need...
